    Required for every collection — DOT and Non-DOT.
  • Employer or CCF authorization
    For DOT collections, bring your employer's testing authorization or the chain-of-custody form (CCF).
  • DOT regulated mode information
    FMCSA, FAA, FRA, FTA, PHMSA, or USCG — needed to complete the Federal CCF correctly.
  • Donor information form
    We'll provide this on arrival for self-pay individuals.

    Medical Review Officer verifies and releases the final result to the employer or donor.

What's the difference between DOT and Non-DOT testing?
DOT testing is federally regulated (FMCSA, FAA, etc.) and uses the Federal CCF with MRO review. Non-DOT testing is for private employers and individuals and is more flexible on panel and process.
How long do results take?
Rapid screens give a preliminary negative on-site in 10–15 minutes. Lab-confirmed results are typically released within 1–3 business days, plus MRO review for DOT.
What's tested in a 5, 10, or 12-panel?
5-panel covers the standard DOT substances. 10 and 12-panel add benzodiazepines, barbiturates, methadone, oxycodone, and others. Ask our team for the current substance list.
How long should I wait before testing after using prescribed medication?
Don't stop or delay prescribed medication. Bring documentation — the Medical Review Officer reviews legitimate prescriptions during result verification.
Can employers set up an account for ongoing testing?
Yes. We work with employers on pre-employment, random, post-accident, and reasonable-suspicion testing programs.
Is testing confidential?
Yes. Results are released only to the authorized parties — the donor, employer, or MRO as required by the testing program.
